Gallstone disease also causes heartburn, but its location is more focused on the upper right abdominal area which spreads to the waist, back, sometimes even to the shoulder.
Gallstone pain symptoms also usually last a long time - can last for hours - and do not improve after taking antacids or other ulcer medicines.

Heartburn and gallstone symptoms can both occur after eating. However, the symptoms of gallstones usually get worse after eating fatty foods such as fried foods, coconut milk, and so on.
Whereas in heartburn, any food, whether fatty or not, will still stimulate the production of stomach acid and cause pain.
Taking antibiotics, NSAID pain medications, and corticosteroids can also trigger heartburn. However, these drugs do not affect the development of gallstone symptoms.

Gallstone disease usually also causes abnormalities in the color of stool and urine. If you have gallstones, the color of the stool is usually lighter while the urine is dark yellow or dark yellow like tea. Heartburn does not cause symptoms of discoloration in either.
In addition, the symptoms of gallstones can cause skin discoloration and the whites of the eyes to turn yellow due to impaired kidney function. Symptoms like this will not be found in stomach ulcers.
