What is Sevelamer Hydrochloride for?

Sevelamer is a drug to lower blood phosphorus (phosphate) levels in patients who are undergoing dialysis due to serious kidney disease. Sevelamer works by binding to phosphate from food so that phosphate can get out of your body.

Reducing the level of phosphate in the blood can help strengthen bones, prevent mineral buildup in the body, and can reduce the risk of heart disease and stroke that can be caused by high phosphate levels.

How do you use Sevelamer Hydrochloride?

Take this medication, usually 3 times a day with meals or as directed by your doctor. Swallow the tablet whole. Do not crush, chew or split the tablet. The dosage is based on your medical condition, other phosphate-lowering drugs you are taking and your response to therapy.

If you are using the powder form of this medication, check the manufacturer's instructions on the packaging for the amount of water to use for a particular dose. Stir and drink the mixture in 30 minutes. If the powder settles to the bottom of the glass, stir it again before drinking.

Use this drug regularly to get optimal benefits. To help you remember, take this medication after each meal every day, or on a schedule given by your doctor.

Do not take other medicines 1 hour before or 3 hours after taking this medicine. Using other drugs during this time can reduce the effectiveness of other drugs. Consult the pharmacist for more information.

How to store Sevelamer Hydrochloride?

This medication is best stored at room temperature, away from direct light and damp places. Don't keep it in the bathroom. Don't freeze it. Other brands of this drug may have different storage rules. Observe the storage instructions on the product package or ask your pharmacist. Keep all medicines away from children and pets.

Do not flush medicines down the toilet or down the drain unless instructed to do so. Discard this product when it has expired or when it is no longer needed. Consult your pharmacist or local waste disposal company about how to safely dispose of your product.

What is the dosage for Sevelamer Hydrochloride for adults?

Sevelamer carbonate: 800 - 1600 mg orally 3 times a day with each meal based on serum phosphorus levels

• Phosphorus above 5.5 and less than 7.5 mg / dL = 800 mg
• Phosphorus 7.5 and above and less than 9.0 mg / dL = 1600 mg
• Phosphorus 9.0 and above = 1600 mg

Hydrochloride Sevelamer:

• Phosphorus above 5.5 and less than 7.5 mg / dL = 1 800 mg tablet or 2 400 mg tablets taken 3 times a day with meals
• Phosphorus 7.5 and above and less than 9.0 mg / dL = 2 800 mg tablets or 3 400 mg tablets taken 3 times a day with meals
• Phosphorus 9.0 and above = 2 tablets 800 mg or 4 tablets 400 mg taken 3 times a day with meals

For patients switching from calcium acetate to sevelamer:

Sevelamer carbonate: For each 667 calcium acetate tablet per meal, provide 800 mg sevelamer carbonate per meal.

Hydrochloride Sevelamer:

• 1 667 mg calcium acetate tablet per meal: Replace 1 sevelamer hydrochloride 800 mg tablet or 2 sevelamer hydrochloride 400 mg tablets at each meal.
• 2 tablets of calcium acetate 667 mg per meal: Replace 2 tablets of sevelamer hydrochloride 800 mg or 3 tablets of sevelamer hydrochloride 400 mg at each meal.
• 3 calcium acetate 667 mg tablets per meal: Replace 3 sevelamer hydrochloride 800 mg tablets or 5 sevelamer hydrochloride 400 mg tablets at each meal.

Patients switching from sevelamer hydrochloride to sevelamer carbonate: Dosage should be given on a gram per gram basis.

The maximum daily dose studied for sevelamer hydrochloride was 13,000 mg and for carbonate 14,000 mg.

What is the dosage of Sevelamer Hydrochloride for children?

An initial dose of 121 plus or minus 50 mg / kg / day (4.5 plus or minus 5 g / day) is used. The dosage was adjusted based on serum phosphorus with a final dose of 163 plus or minus 46 mg / kg (6.7 plus or minus 2.4 gm / day) without any adverse effects (Mahdavi, 2003). In one study of patients aged 0.9 to 18 years with chronic kidney disease, a mean dose of 140 plus or minus 86 mg / kg / day (5.38 plus or minus 3.24 g / day) resulted in good phosphorus control with minimal adverse effects. The initial dose is given based on the previous phosphate binder dose and adjusted according to the serum phosphorus.

Side effects

What are the possible side effects of Sevelamer Hydrochloride?

Seek immediate medical help if you experience any of the following signs of an allergic reaction: hives, difficulty breathing, swelling of the face, lips, tongue, or throat.

Stop using sevelamer and contact your doctor immediately if you experience serious side effects such as:

• Black, bloody stool
• Constipation that gets worse or doesn't get better
• Severe constipation with stomach pain
• Fever, chills, body aches, flu symptoms.

Less serious side effects include:

• Nausea, vomiting, stomach pain, loss of appetite
• Upset stomach, gas, bloating
• Diarrhea, mild constipation
• Feel tired
• Itchy rash
• Joint pain.

Not everyone experiences the following side effects. There may be some side effects not listed above. If you have concerns about certain side effects, consult your doctor or pharmacist.

What should be known before using the Hydrochloride Sevelamer?

Allergy

Is Sevelamer Hydrochloride safe for pregnant or breastfeeding women?

There are no adequate studies regarding the risks of using this drug in pregnant or breastfeeding women. Overdose

What should I do in an emergency or overdose?

In case of an emergency or overdose, contact the local emergency services provider (118/119) or immediately to the nearest hospital emergency department.

What should I do if I miss a dose?

If you forget a dose of this medicine, take it as soon as possible. However, when it is nearing the time of the next dose, skip the missed dose and return to the usual dosing schedule. Don't double the dose.
