Flumazenil has been classified as pregnancy category C by the FDA. Animal studies have failed to reveal evidence of a teratogenic effect. Increased embryonic mortality has been reported in rabbits treated with doses greater than 60 times the maximum recommended human dose. No data on human pregnancy. Flumazenil is only recommended for use during pregnancy when the benefits outweigh the risks. The use of flumazenil during labor and delivery is not recommended.
- Breastfeeding:
There are no data on the excretion of flumazenil into human milk. The manufacturer recommends caution when administering flumazenil to breastfeeding women.
Side effects
What are the possible side effects of Flumazenil?
Get emergency medical help if you have any of these signs of an allergic reaction: nausea, vomiting, sweating, hives, itching, difficulty breathing, swelling of your face, lips, tongue, or throat, or feel like you might pass out.
Stop using Flumazenil and call your doctor if you have any of the following serious side effects:
- Seizures
- Weak or shallow breathing
- Continue to be drowsy for more than 2 hours after receiving flumazenil
- Confusion, fear, panic attacks
- Fast or uneven heartbeat
Less serious side effects include:
- Pain or irritation where the medicine was injected
- Agitation or tremors (shaking)

What is the dosage of Flumazenil for adults?
Usual Adult Dose to Counter the Effects of Sedation
Initial dose: 0.2 mg IV once for 15 seconds.
Repeat dose: 0.2 mg can be repeated every minute until the desired level of consciousness is reached. The maximum total dose is 1 mg. Most patients receive a dose of 0.6-1 mg.
Resedation dose: 0.2 mg every minute up to a total of 1 mg / dose and 3 mg / hour.
Usual Adult Dose for Benzodiazepine Overdose
Initial dose: 0.2 mg IV once for 30 seconds.
Repeat dose: 0.5 mg can be repeated every minute. The maximum total dose is 3 mg. Some patients who receive a dose of 3 mg may receive an additional dose of up to 5 mg. Most patients receive a dose of 1-3 mg.
Resedation dose: 0.5 mg every 20 minutes for a total of 1 mg / dose and 3 mg / hour.

1-17 years:
Initial dose: 0.01 mg / kg IV over 15 seconds.
Repeat dose: 0.01 mg / kg given for 15 seconds; can be repeated 0.01 mg / kg after 45 seconds, then every minute for a maximum total cumulative dose of 0.05 mg / kg. Continuous IV infusion: 0.005-0.01 mg / kg / day used in preterm conditions (gestational age: 32 weeks) exposed to high doses of intrapartum diazepam.
Benzodiazepine-induced myoclonus:
IV:
0.078 mg / kg is effective in a single, full-term neonate receiving continuous midazolam infusion.
