Based on the report of the World Health Organization (WHO) in 2020, Indonesia ranks second as the country with the highest burden of tuberculosis (TB) in the world after India. The estimated caseload incurred per 2019 is 854,000. Meanwhile, for drug-resistant TB (TB RO) cases, Indonesia is 1 in 10 countries that contribute to 77% of RO TB cases globally.
Unfortunately, from the overall estimate, there are still many who start treatment late or do not comply with the rules for taking medication properly.
Community programs are very important to help them reach and start treatment early so that their disease can be treated early on.
A companion community for drug-resistant TB patients in Indonesia
Drug-resistant tuberculosis (TB RO) is a condition when antibiotic drugs for first-line TB are not effective in curing TB infection in the patient's body.
When TB patients develop drug resistance, treatment becomes more complex and takes longer.
The high number of drug-resistant TB sufferers is triggered by a variety of factors. Some of the triggers for TB RO are inadequate treatment methods and patients who are neglectful of treatment. Because it's a role

The role of the community is very important in supporting the diagnosis of TB patients and treatment to increase the success of treatment. Community is an important aspect in realizing a TB-free Indonesia in 2030 which was proclaimed by the Ministry of Health and the world health organization (WHO).
MANDIRI-TB programs from communities and organizations
In supporting the elimination of TB 2030, the KNCV Indonesia Foundation (YKI) is running a project called Mandiri-TB (Network Mobilization for Independence Against TB). This program will be implemented in 4 cities, namely Medan (North Sumatra), North Jakarta (DKI Jakarta), Surabaya (East Java), and Makassar (South Sulawesi).
The Mandiri-TB project is designed to encourage and strengthen the capacity of community organizations and patient organizations in conducting advocacy. Civil society organizations are expected to be active in providing assistance to drug-resistant TB patients in treatment, including psychosocial assistance for RO TB patients.
The Mandiri-TB program will help communities and community organizations that assist TBs to get funding from both the government (APBD) and the private sector (CSR and philanthropy). This program also provides supervision and monitoring on an ongoing basis for the allocation of funding.
In addition, Mandiri-TB aims to strengthen the capacity of civil society organizations and patient communities in conducting counseling on patient behavior change. This includes realizing quality TB services and centering on patient recovery.
"In its implementation, the MANDIRI-TB project will carry out two main strategies. First, through a multisectoral approach to increase the commitment of local governments and the private sector regarding the TB activity budget. Second, increasing access to quality and community-centered TBC RO services, ”explained dr. Jhon Sugiharto, MPH, Director of the KNCV Indonesia Foundation.
This project also contributes to realizing the independence of the TB control program through collaboration with local governments, health service providers, community organizations, and the private sector.
Mandiri-TB will be implemented from the beginning of this year to September 2022. The target is that 100% of RO TB patients in 4 intervention areas can start treatment and 80% of them can complete treatment.
